Beginning June 29, 2024, we have moved to a new Catalyst Physician Group patient portal https://30724-1.portal.athenahealth.com to deliver coordinated care across all Catalyst Physician Group practices and providers as well as personalized communication and access as we co-manage your health journey.

Please create a new account using the same email address on file to take advantage of our online services and continue communicating with your provider for health questions and prescription refills or to view your health records.
